Instructions for Completing the Request for Citation Review Form
It is very important that the Request for Citation Review be filled out completely and accurately for TAPS to process your request. Please contact the Citation Center at (530) 752-7015, or by email if you have questions about how to prepare the Citation Review form. See the example below for filling in fields referenced from the Citation. Appeals submitted as a hard copy will not be returned. Please keep a copy for your records.
